1. No Purchase is necessary to participate in this sweepstakes,
and you do not have to submit to a sales presentation. Void where prohibited by
law, rule, or regulation. Odds of winning are dependent upon the number of
entries received. Winners will be selected in a random drawing from among all
entries received. Melani Bros., Inc. will conduct the drawing on or about
January 15, 2009. To enter, complete and return this Official Entry Form.

received by December 31, 2008. Mechanically reproduced entries will not be
accepted. 2. Grand Prize (Two (2) Winners): Choose either (a) windows for the entire
home (limit 15 double-hung or sliding windows, est. retail
value $12,000); (b) a Melani Bros. Sunroom (10' X 12', est.
retail value $25,000); (c) an entry door system (est. retail
value $1,700); (d) a 10' X 10' deck (est. retail value
$3,000); (e) a gutter protection system (est. retail
value $1,700); or (f) maintenance-free siding (est. retail value $15,000). First Prize (Two
(2) Winners):
Winner's choice of one (1) bay,
bow or garden window (est. retail value $5,000 each). Second
Prize (One (1) Winner): sliding patio door (est. retail value $2,500). Third Prize (Fifty-One
(51) Winners): Choose either (a) one (1) double-hung window (est. retail value
$590); (b) 1 (one) sliding window (est. retail value $590); (c) a $200 donation
to the Children's Miracle Network; or (d) a $100 cash prize. All prizes include installation.
All prizes will be awarded. All expenses not specifically provided for are the
sole responsibility of the winners. 3. Melani Bros., Inc. reserves the
right at its sole discretion to suspend, modify, or terminate this sweepstakes
without notice at any time. Prizes are subject to availability and may be
subject to black-out dates. Winners will be notified by telephone. In the event of a
dispute concerning the identity of the person submitting an entry, the entry
will be deemed to be submitted by the person in whose name the winning home is
titled. Limit one (1) prize to
an individual or household. Prizes are nontransferable and
no substitutions are allowed. Melani Bros., Inc. assumes no
responsibility or liability for damages, losses or injury
resulting from acceptance or use of any prize. Taxes, if
any, are the responsibility of the individual winners.
Any required tax reporting forms will be furnished to the Internal Revenue
Service. Winners must take delivery of prize within one (1) year of being
notified by Melani Bros., Inc. In the event the winner has
purchased a product that is all or part of their prize
between July 1, 2007 and December 31, 2008, such prize winner may elect
to receive the retail value of that prize in lieu of the
product or products. Winners may be required to verify
address or execute an affidavit of eligibility and release
within fourteen (14) days of notification attempt or prize will be
forfeited and an alternate winner selected. Entry
constitutes (a) permission to use winners' names and
likenesses for promotional purposes without further
compensation except where prohibited by law, and (b)
permission to be called for advertising purposes. No
responsibility is assumed for lost, misdirected, illegible
or late entries or mail. 4. Sweepstakes entrants must (i) be legal residents of
Virginia or North Carolina, (ii) be 18 years of age or older at the time of
entry, (iii) own their own home, which home is in a suitable condition (as
determined by Melani Bros., Inc.) for installation of Melani Bros., Inc.
products (including compliance with local building codes and regulations), and
(iv) reside in the geographic area served by Melani Bros., Inc. A listing of
communities served by Melani Bros., Inc. can be found at
http://www.melanibros.com. Employees, officers, and directors of Melani Bros.,
Inc., its affiliates, subsidiaries, co-branded partners, consultants,
contractors, advertising, public relations, promotion, fulfillment, and
marketing agencies, and their immediate families (and those living in their same
households) are not eligible in this sweepstakes. 5. By participating in
this sweepstakes, participants agree to release, discharge, and hold harmless
Melani Bros., Inc., its affiliates, co-branded partners, subsidiaries,
retailers, agencies and their respective officers, directors, employees, and agents from
any damages that may arise out of participation in this sweepstakes in any way
or out of the acceptance, use, misuse, or possession of a prize. By entering
this sweepstakes, participants agree that any and all disputes, claims, or
controversies (hereafter referred to as a "Claim") arising under or relating to
this sweepstakes, including the validity or enforceability of this arbitration
provision, shall be subject to binding arbitration to be determined by one
arbitrator, in accordance with and pursuant to the then prevailing Code of
Procedure of the National Arbitration Forum ("NAF"), to be held and arbitrated
in the judicial district in which the participant lives at the time of entry.
Participants agree they will not assert a Claim on behalf of, or as a member of,
any group or class. The findings of the arbitrator shall be final and binding on
all parties. Each party shall be responsible for its own fees and costs. This
agreement to arbitrate, and any award or verdict will be specifically
enforceable under the prevailing law of any court having jurisdiction. Notice of
the demand for arbitration will be filed by the party asserting the Claim with
NAF. The demand for arbitration shall be made within a reasonable time after the
Claim in question has arisen, and in no event shall any such demand be made
after the date when institution of legal or equitable proceedings based on such
Claim would be barred by the applicable statute of limitations. Any arbitration
proceedings and any award or verdict shall remain confidential between the

sweepstakes is conducted in English (which will be given its everyday
